Obesity Medicine Physician - Part-time (Remote)
Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Obesity impacts more than 40% of the US adult population, and although historically only about 1% of patients received medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth. Form Health provides high-quality expert care and leverages technology to enhance the patient experience. All Form Health patients work closely with their physician-led care team, which also includes advanced practice professionals and Registered Dietitians. Through our proprietary mobile app patients engage in regular video visits, as well as text messaging, photo journaling, digital data transmission, and customized educational materials. We are seeking Physicians specialized in Obesity Medicine to join our innovative, multidisciplinary chronic disease management team. A valid medical license to practice and prescribe legally in any US state is required, and eligibility to obtain additional state licenses as needed is required.
This role is designed for an American Board of Obesity Medicine (ABOM) certified Physicians with demonstrated expertise in obesity medicine, family practice or internal medicine, and a strong interest in working collaboratively with Obesity Medicine Advanced Practice Providers (APPs) in providing high-quality patient care.
This is a fully remote, W2 part-time position (US-based), with a minimum of 20 hrs/week of total work, including 16 hours of clinical patient-facing time and 4 hrs of administrative time.

Full-time physician postings describe a fully remote W-2 position based in the US, with a choice of 32, 36 or 40 total hours per week, split roughly 80% clinical, patient-facing work and 20% administrative work.
